The patient portal allows patients to enter demographic information, medical information, and acknowledge required documents online through a secure login. It allows information to be exchanged securely and in full compliance with HIPAA.
The Patient Portal allows patients to provide information from the comfort of their own home and reduce time during their office visit.
The patient portal also allows new and established patients to request appointments online!
Patient Portal Information
The following information will be needed when registering on the patient portal: Name, Date of Birth, Social Security Number, Sex, Address, Phone. Patients must have a valid email address and choose a password. The patient policies must be accepted to create a portal account.
After creating a login, please provide the following information with as much detail as possible.
Use the left hand menu or the icons to manage information.
Required Portal Information
Add Emergency Contact
Click Add new medication and complete as much information as possible. Repeat for each medication
Enter allergy information
Enter the pharmacy where you prefer to fill your prescriptions
Enter surgery history
Update required fields
Check the box for "patient does not have any insurance"
Add primary care doctor & specialists seen on a regular basis

Click the forgot password link on the portal or you may call the clinic
What if I need to update my information?
You can always log into the portal to update your demographic and medical information.
How do I make an appointment?
Once logged in to the portal, scroll down to the communications section and click on "request appointment"

The information entered into the portal is secure and HIPAA compliant.
